PROGRAM:             Community Health Representatives

SERVICES:          Community Health Representatives Program- Provides home visits to assigned patients and transportation to medical appointments for patients of the Health Center who have no other means of transportation. All transportation requests must be made in advance and are on a first come basis.
 

PROGRAM:              Contract Health Services

SERVICES: The Contract Health Services Program purchases health services, which cannot be provided at the Keweenaw Bay Indian Community Department of Health and Human Services.  This program is not an insurance company nor an entitlement program.  Payment for services under this program depend on the availability of Federal funding and medical priority.  You are strongly encouraged not to rely on Contract Health funds as your insurance coverage.  The services provided include:

  • Emergency Room Care for emergent/urgent medical condition, surgical conditions or acute trauma.

  •  Emergency Inpatient Hospital Care for emergent/urgent medical conditions, surgical conditions or acute injury that   would result in immediate death, loss of limb or senses or result in the serious impairment to the health of the individual.

  •  Renal replacement therapy, acute and chronic.

  •  Emergency psychiatric care involving suicidal persons or those who are a serious threat to themselves or others.

  •  Obstetrical deliveries, prenatal care and acute perinatal care.

  •  Routine and medically indicated eye examinations.

  •  Eye glasses

Most services require a written referral from the Tribal Health Department Physician.  The referral does not ensure payment.  Please call the Contract Health Services Office at the above number for questions.

 

PROGRAM:                 Dental Clinic

SERVICES: Provides dental and hygienist services to qualified Native Americans at the Donald A. LaPointe Health and Education Center.  This includes check-ups, hygienist appointments and other dental work.  Dental visits are mainly by appointment.  Minimal walk-in appointments are available.  Emergency appointments may be arranged by calling (906) 353-8787

Dental Clinic Hours:  Monday-Thursday: 7:00 a.m.-5:30p.m.

 

PROGRAM:                   Diabetes Program

SERVICES:

Diabetes Education—Individual diabetes education for type 1, type 2, and gestational diabetes is available upon request from patient or by referral from a physician.

Diabetic Supplies—Blood glucose monitors and lancet devices are available.  Test strips are provided to those with no insurance for a $4.00 co-pay.  Anyone with insurance is asked to use this resource, so that we may continue to provide patients without any insurance their strips.

Nutrition Consults—Nutrition consults are available through the diabetic program.  Consults are available to any Native America.

Diabetic Labs—All diabetic labs, required by Indian Health Service Standards of Care for Type 2 Diabetes, will be obtained at the required intervals during diabetic clinic.  Covered in-house labs include, lipid panel with glucose, ALT,A1C, Microalbumin.  Labs covered at GCMH are creatinine, TSH, and if necessary a random spot microalbumin.   Appointments for the diabetic clinic may be made by calling (906) 353– 8700.  Diabetic Clinic is presently being held on Tuesday & Thursday AM.  Native American diabetics with no insurance will have their labs that are required for Indian Health Service Diabetic Standards, paid for through the diabetic program, al long as funds are available.

Exercise Physiologist—provides exercise classes.

Lunch & Learn—Lunch & learn programs are sponsored by the diabetic program.  Lunch is provided free of charge.  To reserve a spot and to find out more about upcoming programs, please call.

Weight Loss Program—Thursday classes, along with weigh in, are available.  Call to reserve a spot as space is limited.

Diabetes Support Group—Held at BCMH every 3rd Wednesday of the month from 7-8 p.m., co-sponsored by KBIC/DHHS Diabetes Program.  

ELIGIBILITY:  All Native Americans.

 

PROGRAM:            Domestic Violence Prevention

SERVICES: Domestic Violence Program- Provides intervention and counseling to individuals who have committed domestic violence against others. Contact program for details.

 

PROGRAM:            Healthy Start

SERVICES: The Healthy Start Program provides pre-natal and post-natal care to Native American women, as well as health assessments to Native American infants and children up to the age of two.  Healthy Start provides office or home visits by a Maternal Child Health Nurse, community health promotion and risk reduction events, as well as referrals, follow-up and afar-care services.  There are no income guidelines to determine eligibility.  Contact the Healthy Start Program for further details. 

 

PROGRAM:            Medical Clinic

SERVICES: Medical Clinic-Provides ambulatory care to qualified Native Americans accessing the Donald A. LaPointe Health and Education Center.  The Medical Clinic is staffed with two full time Board Certified Family Practice Physicians, and a Part time Board Certified Pediatrician.  Routine medical care, as well as acute and long term care, is provided by the physicians.   Physicians also provide inpatient hospital care at the Baraga County Hospital.  The medical clinic features six exam rooms as well as a procedure room and a small lab.  The main nurses’ station is equipped with a computer that has direct access to the Michigan Childhood Immunization Registry to aide with the Pediatric Clinics.  The Lab has equipment available to do onsite testing for Diabetes, Kidney & Liver Disease, basic testing for acute illness and blood draws.  Diabetic & Immunization clinics are held weekly.  Scheduled appointments are encouraged, but walk in appointments are also available for acute care needs.
